Four Peptides, Four Different Stories
How BPC-157, GHK-Cu, KPV, and semaglutide differ in chain length, origin, mechanism, and — most importantly — how much of the evidence behind each one comes from humans.
The short version

Chain length and origin
The four peptides span almost the entire useful size range for this class of molecule. KPV and GHK-Cu sit at the short end, just three amino acids each, short enough that GHK-Cu can wrap around and chemically bind a copper ion within its own chain, and short enough that KPV is really just the tail fragment of a much bigger hormone rather than a peptide designed from scratch. BPC-157, at 15 amino acids, is a middle-length fragment cut from a gastric protein. Semaglutide, at 31 amino acids, is the longest and the only one that was deliberately engineered rather than isolated or fragmented — researchers started with the natural hormone GLP-1 and made two targeted substitutions plus added a fatty side-chain, rather than simply cutting a piece out of something bigger.
Evidence base
This is where the four separate most sharply. Semaglutide's evidence base is built from large, multi-thousand-participant randomized controlled trials, SURMOUNT-5 (n=751), FLOW (n=3,533), SELECT (n=17,604), and STEP 1 (n=1,961) among them, plus years of post-marketing safety surveillance [18][19][20][21]. BPC-157's human evidence, by contrast, consists of exactly three small pilot studies as of a 2025 review, with everything else drawn from rats and dogs [1][2]. GHK-Cu occupies a middle position: its topical, cosmetic use has real controlled human trials behind it, including a 45-person hair-growth RCT [10], but its injectable or systemic use has essentially no human data at all. KPV has the thinnest record of the four — every study is either a cell-culture experiment or a mouse model, with zero published human trials of any kind [13][14][15][16][17].
Regulatory status
Semaglutide is the only one of the four that is an approved medicine, available by prescription for type 2 diabetes, chronic weight management, cardiovascular risk reduction, and, since 2025, MASH. GHK-Cu occupies a split status: as a topical Copper Tripeptide-1, it's a legal, widely marketed cosmetic ingredient; as an injectable or oral systemic product, it has no approved status and no regulatory pathway. BPC-157 and KPV are both unapproved research chemicals in every form, sold under 'not for human consumption' labeling, with no established human dose, and, in BPC-157's case, a specific 2023 FDA determination that it isn't eligible for pharmacy compounding under current rules.
Key caution
Each peptide carries a defining caution. For BPC-157, it's that the human evidence is extraordinarily thin, three small pilot studies against a large rodent literature, much of it from a single research group [2]. For GHK-Cu, it's the gap between its well-documented topical safety record and its essentially unstudied injectable use. For KPV, it's the starkest of all: no published human trials exist at any dose, by any route, for any use. For semaglutide, the cautions are different in kind, not 'is this studied,' but specific, cited clinical-trial findings: gastrointestinal intolerance as the leading cause of discontinuation, a boxed warning for thyroid C-cell tumors, and substantial weight regain after stopping [22]. Reading all four together, the throughline is the same: chain length and mechanism tell you what a peptide does; regulatory and trial history tell you how much to trust what's claimed about it.
